Important Notes
The Alliance has processed thousands of Credentialling applications. This section provides some important notes about the Credentialling process to help you avoid common mistakes made by applicants. Remember that every mistake slows down your Credentialling process.
Be sure to read the Credentialling Package thoroughly for understanding before applying. Click here to download a Credentialling Package
Translations
If your documents are in a language other than English or French, they must be translated word for word into English or French and notarized as a certified translation by a notary public or commissioner of oaths. Notarized certified translations must be sent to The Alliance with notarized copies of the documents in their original language.
Document received from your school in a language other than English or French will be sent to you for translation before the assessment starts. Documents from your school must be mailed directly to our office from the school.
Notarized Documents
Notarized documents are signed by a person, often called a notary public, to validate that the documents are real. A notary public is authorized by the state/country in which the person resides to administer oaths (swearing to truth of a statement), and certify documents. The signature and seal or stamp of a notary public is necessary to confirm the oath of truth of the person executing the document.
The Alliance accepts documents notarized by a notary public or commissioner of oaths.
If your documents are not original, then they must be notarized. Only documents submitted that are notarized will be accepted. For example if you have your documents signed by your doctor, they will not be accepted. Or, if your friend is a notary public and signs it, but forgets to put a seal on the document, your documents will not be considered officially notarized.
Transcripts
Please remember that The Alliance needs transcripts from all of your university level educational programs. If you attending several universities, transcripts must be sent directly to The Alliance from each of those universities.
Document Request Forms
The Alliance may already have copies of the program’s course descriptions and outlines (e.g. syllabus, course descriptions) that can be used in the assessment process.
Please contact a Credentialling Officer before sending the Document Request Form to your school to confirm if the syllabus is required from your school.
Authorization Letter
If you are to be represented by another person, you must provide to The Alliance an Authorization Letter that has been signed by you, notarized in the country where you reside by a notary public or commissioner of oaths. Click here to download a copy of the Authorization Form.
Please note that we need your original signature on this form, so it must be sent by mail.
Language Testing
Remember that you will need to take a language proficiency test if your physiotherapy education is not in English or French. At least 80% of your physiotherapy education must be in English or French including patient interaction during your clinical supervised practice.
Application Form
Remember to read the Consent/Waiver thoroughly for understanding. You must sign and date your application form before sending it to The Alliance.
The Alliance will not accept an application that is not personally signed by the Applicant.

Acknowledgement of Application
If you would like to know when your application and documents are received, please send them by Registered Mail, Canada Express Post, or through a courier service that gives you confirmation (e.g. Federal Express, United Parcel Service).
You may also complete the Confirmation Form (Form A) found in the application package. We will complete the Form and return it to you within five (5) business days.
