Document Checklist

The documents listed below are needed to process your application. Some documents are required from you and others must be sent directly from your school. Additional documents or information may be requested in support of the application.

Please Note: Incorrectly submitted documents will delay the evaluation of your application.

Signed and Dated Application Form

Remember to read through the application before signing and dating it. If you have any difficulty understanding any part of the application, please contact a Credentialling Officer. Please note that The Alliance has two Credentialling Officers: one handles assessments of Applicants with last names beginning A-K. The other handles assessments of Applicants with last names beginning L-Z.


Assessment Fee

Please submit the fee in Canadian currency payable to "Canadian Alliance of Physiotherapy Regulators".

We do not accept currency other than Canadian dollars. All payments made outside Canada must be made in Canadian funds by international bank draft or money order drawn on a Canadian bank. You may also pay by credit card if you wish. Click here for payment options.

Payments within Canada may be made by certified cheque, money order or credit card.

Cash payments will not be accepted. If your payment is returned by our bank, you will be charged an additional $50.00.


Notarized Copy of Birth Certificate

Your birth certificate must be certified as a true copy of the original by a notary public or commissioner of oaths. If your birth certificate is not in English or French, please attach a certified translated copy of the birth certificate. A "birth certificate" issued by a consulate will not be accepted.

If you do not have an official document called a birth certificate, you must personally make a statutory declaration before a notary public or commissioner of oaths stating that the name shown on your application is the one given to you at birth, your date of birth and the reason why you do not have a birth certificate. You must submit certified copies of two (2) official documents bearing that name and birth date in support of your declaration. Acceptable documents include a social insurance card, passport, driver's license, or Immigration Record of Landing.

The names on your supporting documents must be spelt exactly the same as in your declaration.


Notarized Copy of Marriage Certificate

Your marriage certificate must be certified as a true copy by a notary public or commissioner of oaths. If your marriage certificate is not in English or French, please attach a certified translated copy of the marriage certificate.

This document is only necessary if you have officially changed your name after marriage and wish to use your married name, or if any of your official documents (degree, transcripts, etc) are issued under your married name.

If the name on your Application Form is different from the one on your birth or marriage certificates, you must submit a certified copy of your legal change of name document.


A copy of your Passport

Please provide only those pages that have your picture, full name and passport expiration date.


Two Passport Photographs

Your photographs must be taken within the last six months, signed and dated by the applicant.


Evidence of Language Proficiency in English or French

If your physiotherapy education was not in English or French, language proficiency tests are required by The Alliance to prove your fluency in English or French. The Alliance accepts the following language proficiency tests:

The original language proficiency score sheet must be mailed to our office directly from the issuing institution. Proficiency score sheets that do not come directly from the issuing institution will not be accepted by The Alliance.


Membership/Registration/Licensure Statement

Please notify the Licensing Authority in the country where you studied physiotherapy. The Alliance requires a currently-dated letter to be mailed to our office directly from the Licensing Authority, stating that you are currently licensed as a physiotherapist. If the information / letter is not mailed directly by the licensing authority to The Alliance office it will not be accepted by The Alliance.

If you are not currently licensed as a physiotherapist (e.g. you are a recent graduate), please contact the regulatory authority, or organization authorized to license you to practice, for a letter confirming your eligibility to register to practice as a physiotherapist.

In some countries you must successfully complete a licensing examination to be able to practice. If this is the case, please contact the examination authority for a letter confirming your eligibility to apply for the examination.

For clarification, please contact a Credentialling Officer.

Document Request Form

You must send the Document Request Form to your physiotherapy school. Only complete the indicated section on the first page of the form. Your physiotherapy school must complete all other sections of the form.

The Document Request Form asks your physiotherapy school to provide The Alliance with the following:

  • Transcripts/mark sheets/grade lists/records
  • Corresponding syllabus/course descriptions from your physiotherapy school
  • Corresponding clock hours for all courses completed during your physiotherapy education

If you are a graduate of a physiotherapy program in India or Pakistan, your mark sheets (grade lists / records) must be submitted directly from the University authorized to issue mark sheets. The mark sheets submitted by the college will not be accepted. All other documents listed on the Document Request Form may be submitted by the College.

The Document Request Form and supporting documents must be mailed to our office directly from the issuing institution. The Alliance will not accept any Document Request Forms or supporting documents that do not come directly from the issuing institution.

The Alliance may already have copies of the program's course descriptions and outlines (e.g. syllabus, course descriptions) that can be used in the assessment process. Please contact a Credentialling Officer before sending the Document Request Form to your school to confirm if the syllabus is required from your school.


Notarized Diplomas and Certificates

Please submit a copy (certified as a true copy of the original by a notary public or commissioner of oaths) of your original language diploma(s)/certificates from high school/secondary school and university level educational programs. If your diplomas or certificates are not in English or French, you must submit a certified translated copy of the documents.


Notarized and translated copy of any final thesis or major research paper

The Alliance only requires the cover page, table of contents, abstract and bibliography/reference list.

If your final thesis or research paper is not in English or French, please attach a certified translated copy of the pages requested above.


Current resume

The Alliance only requires a current resume.


Authorization Form

If you are to be represented by another person, you must provide to The Alliance an Authorization Letter that has been signed by you, notarized in the country where you reside by a notary public or commissioner of oaths. Click here to download a copy of the Authorization Form.
