Changing Your Information

You may need to change some of your information after you apply for Credentialling.

Changing Your Address

If you move, you must give us your new address in writing. Use the Change of Information Form. Be sure to tell us when the change takes place. We will mail your results and any other correspondence to the address we have in our files.

Changing Your Name

If you change your name after you apply for credentialling, you must give us your new name in writing. Use the Change of Information Form. You need to send a notarized copy of a name change document with the form (for example, your notarized marriage certificate). We cannot change your name after we send your results.
