Applying for an Assessment

Follow these steps to apply for Credentialling.

1. Read the Credentialling Handbook.
2. Make sure you are eligible to apply. Check the Credentialling Handbook for information about eligibility. This information is in the section called "Educational Credentials and Qualifications Assessment".
3. Print the credentialling application form and fill it in. Be sure to complete all the fields.
4. Complete the credit card section of the application OR include a certified cheque, money order or a bank draft with your application.
5. Read the Important Notes on the back of the application form, then sign and date the form.
6. Read the information about the documents you and other institutions must send.
7. Put your application and payment in the mail.

If you want us to verify your credentialling status with a regulator, print and complete the Verification Request Form. You can send this when you have received your successful credentialling results letter.
